What voltage do I need to supply?

This is probably a stupid question but I can't figure out what voltage inputs I need to supply the LCD.

Just 5VDC? 5 & 3.3VDC?

Does the built in LCD controller supply the negative voltage?

Do I have to build an external circuit for back lighting?

I believe the CFAG320240CX-YYH-T only needs 5v.
