I can identify manufacturer of this LCD.
Greetings a few years later,
I have just butchered an old Xerox inkjet printer, and my LCD module has part number UMSH-3061ANV-W. This one has 10 connections: contrast, +5V, 2 grounds and 6 data pins so presumably it is parallel 4-bit data. Each of the 6 data pins is buffered through one gate of a 74HCT04 hex inverter so they are all inputs only.
I am trying to find docs on it so I can use it, and so far I have dug up the following:
- The manufacturer is U.R.T. - that's United Radiant Technology Corp in Taiwan.
- The info on how to use this is not exactly falling into my lap. The manufacturer's web site lists what controller chip(s) they used in a bunch of similar displays, but no other help yet.
- here's the manufacturer's web page with info on what the part numbers mean. Based on the similarity of part numbers, yours appears to be a graphic module, while mine is a 20 x 2 array of 5 x 8 dot matrix characters.

You will see that they used a bunch of different controller chips (20!). I have made an alphabetical list:
